About the Role Our client, a national leader in providing integrated healthcare solutions through cutting-edge telehealth technology, is seeking a dedicated and experienced Telehealth Registered Nurse (RN) with a specialization in Case Management. This fully remote position offers a unique opportunity to impact patient care by coordinating services, advocating for patient needs, and ensuring continuity of care from a distance. The ideal candidate will possess strong clinical skills, exceptional communication abilities, and a thorough understanding of case management principles. You will work closely with patients, healthcare providers, and insurance companies to develop and implement comprehensive care plans, optimize health outcomes, and improve patient satisfaction. This role is crucial for bridging gaps in care and ensuring seamless transitions for patients across the healthcare continuum. Key Responsibilities Conduct comprehensive patient assessments via telehealth platforms to identify needs and care goals. Develop, implement, and monitor individualized case management plans in collaboration with patients, families, and healthcare teams. Coordinate patient care across various settings, including hospitals, rehabilitation facilities, home health, and physician offices. Advocate for patients' needs and ensure they receive appropriate and timely healthcare services. Educate patients and families about their conditions, treatment options, and available resources. Facilitate communication between patients, providers, payers, and community resources. Monitor patient progress and outcomes, adjusting care plans as necessary. Maintain accurate, detailed, and timely documentation in the electronic health record (EHR) system. Identify opportunities for process improvement in telehealth case management services. Adhere to all relevant healthcare regulations, including HIPAA. Requirements Associate's Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred. Current and unrestricted Registered Nurse (RN) license in the United States. Minimum of 3 years of experience in nursing, with at least 2 years in case management, care coordination, or a related field. Experience with telehealth platforms and remote patient monitoring technologies. Strong understanding of healthcare systems, insurance processes, and community resources. Excellent critical thinking, problem-solving, and decision-making skills. Exceptional interpersonal, verbal, and written communication skills. Proficiency in using EHR systems and other healthcare-related software. Ability to work independently, manage time effectively, and maintain productivity in a remote setting.
